Registered Nurse Consultant
ChildNet Youth and Family Services
ChildNet Youth and Family Services, Inc. is currently seeking a contract Registered Nurse Consultant, to provide medical oversight and consultation for cases involving complex or high-risk health needs of children and adolescent clients within our Enhanced Care Management (ECM) program located in Riverside, CA.
This is a contracted position (hybrid – mostly remote, minimal onsite work) for a Registered Nurse to serve as a clinical resource, ensuring that members with significant medical or medication-related concerns receive coordinated, safe, and high-quality care. Working within the scope of their license, the Nursing Consultant reviews care plans, offers guidance on clinical decision-making, supports adherence to medical best practices, and collaborates with licensed clinical leadership to ensure compliance with Medi-Cal and DHCS requirements. This role focuses on staff support and quality improvement rather than direct member contact.
Within agency guidelines and professional standards, the Registered Nurse Consultant functions independently and works directly with the ECM Lead Care Managers (LCMs), supervisors, and the Inland Empire Regional Director.
Duties
- Provide nursing oversight for ECM clients with complex or medically specific needs to ensure appropriate coordination and follow-up.
- Serve as a consultative resource for Lead Care Managers regarding medication management, chronic disease coordination, and communication with healthcare providers.
- Review care plans and documentation for medical appropriateness and adherence to nursing and ECM standards.
- Support program leadership in developing and maintaining workflows for medical consultation, escalation, and quality assurance.
- Collaborate with ECM Clinical Supervisor and program leadership to identify trends, training needs, and opportunities for improvement in health-related service delivery.
- Assist with the interpretation of medical information to guide care management activities and interdisciplinary collaboration.
- Provide staff coaching on health-related topics (e.g., chronic conditions, medication adherence, hospital discharge planning).
- Participate in multidisciplinary meetings to review cases and offer recommendations for care coordination and risk mitigation.
- Maintain up-to-date knowledge of Medi-Cal, CalAIM ECM, and Department of Health Care Servcies (DHCS) clinical guidance relevant to nursing and population health.
- Support compliance activities related to medical documentation, quality review, and reporting.
- Perform other duties as assigned to support overall ECM program quality and effectiveness.
Job Requirements
Qualifications
- Associate’s degree in Nursing (ADN) or Bachelor’s Degree in Nursing (BSN).
- Licensed Registered Nurse (RN) with an active, unencumbered license in California.
- Minimum of 3 years’ experience in care management, population health, or community/public health nursing.
- Demonstrated knowledge of complex medical conditions, chronic disease management, and interdisciplinary care coordination.
- Strong understanding of Medi-Cal, CalAIM, and ECM program requirements preferred.
- Excellent communication, organization, and problem-solving skills.
- Ability to work collaboratively with clinical and non-clinical team members to support high-quality service delivery.
- Willingness to travel locally as needed to support team meetings, trainings, and program oversight.
- Bilingual/Spanish desirable, but not required.
- General computer skills in Microsoft Office Suite (Word, Excel, etc.), electronic health care records (EHR: Exym), and familiarity with medical records management.
